糖尿病患者への降圧薬は何が良い？
以前書いた記事を誤って消してしまったため再度。

結果
・全死亡
β遮断薬のみ死亡リスクが有意に高かった(オッズ比7.13、95%信頼区間1.37-41.39)。
以下β遮断薬との比較
・ACE阻害薬＋Ca拮抗薬→オッズ比0.067(95%信頼区間0.008-0.559)
・ACE阻害薬＋利尿薬→オッズ比0.121(95%信頼区間0.020-0.658)
・ACE阻害薬→オッズ比0.137(95%信頼区間0.023-0.711)
・プラセボ→オッズ比0.140(95%信頼区間0.024-0.732)
・Ca拮抗薬→オッズ比0.145(95%信頼区間0.025-0.728)
・ARB→オッズ比0.153(95%信頼区間0.025-0.793）
・末期腎不全
全ての比較で有意な差は見られなかった。
・血清クレアチニン値の倍増
ACE阻害薬ではプラセボ(オッズ比0.58、95%信頼区間0.32-0.90）とβ遮断薬(オッズ比0.12、95%信頼区間0.02-0.74）)と比較して有意にリスクが低下していた。
その他の比較では有意な差はみられなかった。
感想
ACE阻害薬が第一選択薬、もう1剤追加するならCa拮抗薬が推奨されるような結果だが、β遮断薬でなければどれもあまりり差がないように感じる。
まずは安い薬から開始するというような感じでも良いのかもしれませんね。
